+ How many people have you had sex with?
+ Do you have sex with girls, boys, or both?
+ Do you have vaginal sex?  
+ Do you have oral sex? Giving or receiving?
+ Do you have anal sex? Giving or receiving?
+ Do you use condoms? When do you use them?
+ When was your last period; how often do you have them; and how long do they last?
+ Do you have vaginal discharge, spotting, or bleeding between your periods?
+ If you are sexually active, what kind of birth control do you use?
+ Have you experienced any pain or bleeding during oral, anal, or vaginal sex?
+ Have any of your relatives had gynecological problems?
+ Have you had any other kinds of medical problems?
+ Have you ever been pregnant; do you think you might be pregnant; and are you trying to get pregnant?
+ Have you ever had a sexually transmitted disease; do you think you might have one?
+ Any allergies?
+ Do you drink, smoke, or take drugs?
